If at any time you have used a credit card to fund your DSI account, and are requesting a payout, you will be required to complete and fax a Credit Card Authorization Form and documentation for each card used. If you have not already submitted this form and need a copy, click here.

All payout requests may take up to 7-10 business days to be processed.

All withdrawals must go back to the same method used to deposit. (Restrictions apply).
